To Register as a Patient :-

 

 

Each of the Doctors at Bolton House have personal patient lists.  You will only see another Doctor, if your own Doctor is on leave. We will allocate you to one of the Doctors within the Practice. If you wish to register, and we have agreed to take you on our List, you will need to complete a New Patient registration form which asks for details of your past medical history. You will also be asked to attend for a New Patient Check with the Nurse. Please note Patients are not entitled to use the car park attached to the Surgery when attending for appointments, this car park is for the use of Staff only.

Rights & Responsibilities :-

Just as you have the right to access the services we provide without discrimination, or prejudice by us, you also have responsibilities as a patient of the Practice.

We support the Government's NHS Zero Tolerance Campaign against violence & abuse. Our GP's and staff have the right to care for others without fear of being attacked or abused. In the event of such behaviour on the premises, the Police will be called and action will be taken to remove the offender from the Practice List. Equally, abuse over the telephone will not be tolerated.

We will not tolerate racial, gender or religious prejudice within the Practice.

You are responsible for keeping your appointment once booked. Failure to do so, without giving us notification, will result in us writing to you and ultimately you may be removed from our List.

If you are due to receive results from your Doctor, either from a blood test, or a specimen that was sent off to the Hospital for examination, please remember it is your responsibility to call us after 5 working days to access the result.

New Patient Checks :-

New Patient Checks are offered to all new patients over the age of 5, registering at the Surgery. You should bring a specimen of urine with you at your New Patient appointment, for the Nurse to test.

Home Visits :-

If you need a home visit, please telephone the Surgery on 730537 BEFORE 10.30 am.  This will help the Doctor plan his rounds and avoid unnecessary delays. Please remember, however, that house-calls are time consuming and that doctors can usually see at least 4 patients in the time taken to visit at home. Therefore, it is expected that you should attend the surgery where there are better facilities for examination and treatment, unless you are housebound or too ill to go out.

Telephone Advice :-

Sometimes a telephone call will avoid the need for consultation will avoid the need for a consultation. If you wish to speak to your Doctor, please telephone the surgery and the Receptionist will take a message from you and send this to the Doctor for him to return your call. Alternatively, you may be given a time to call back, when it is convenient to be put straight through to your own Doctor.

We have special arrangements for HGV, PSV, Elderly Insurance Driver and Employment medicals. Please ask the Reception when it is convenient to book an appointment for the same.

Sickness Certificates :-

You do not require a Doctor's certificate for illness lasting 7 days or less. You can self-certificate for sickness up to 7 days.

For illness lasting longer that 7 days, you will need to see your Doctor to obtain a sickness certificate. Please note these cannot be done over the telephone, by signing the certificate (Med3), your Doctor is confirming that he has seen you with an illness, therefore an appointment has to be made.

Access for the Disabled :-

There is a ramp for wheelchairs or disabled patients between the car park and the main surgery entrance. Please use the bell for further assistance.

Repeat Prescriptions :-

Prescriptions for medications taken regularly can be obtained without seeing the Doctor, these are called repeat medications. Requests must be in writing, i.e. completion of the repeat slip on the back of the prescription, a letter to the Doctor, or completion of a form at Reception. We do NOT take prescription requests over the telephone.

You will need to allow 2 clear working days for the prescription to be processed !!

Please note if there is an alteration on your repeat slip, or you are requesting additional medication not listed on the repeat slip, your prescription will take longer to process.

We can send your prescription direct to a nominated chemist, we can send the prescription to your home address if you have provided a stamped addressed envelope with your request or you can collect the prescription direct from us.

PRESCRIPTION CHARGES - some patients are automatically exempt from prescription charges - namely children under 16, under 19 if in full-time education, patients over the age of 60, pregnant women and women who have delivered a baby in the last 12 months, patients on DHSS benefits and some patients with specific medical conditions.

REMEMBER IT IS YOUR RESPONSIBILITY TO ENSURE YOU HAVE ENOUGH MEDICINES
