YOU CAN  BOOK APPOINTMENTS and REQUEST REPEAT PRESCRIPTION ITEMS ONLINE BY CLICKING THE LINKS.  PLEASE NOTE YOU WILL NEED A USERNAME AND PASSWORD TO ACCESS THIS SERVICE, WHICH IS OBTAINABLE FROM THE SURGERY - YOU WILL NEED TO REQUEST THIS IN WRITING AND INCLUDE A CURRENT EMAIL ADDRESS FOR THE REPLY. ALTERNATIVELY YOU CAN EMAIL BOLTON HOUSE AND MAKE A REQUEST.  PLEASE MARK YOUR REQUEST FOR THE ATTENTION OF MRS DEBBIE JOLLY.

Please be aware that online booking of Appointments may be used for arranging routine appointments with your Usual GP.  If you use online booking to arrange an appointment with another GP, this may be moved or cancelled.  Thank you for your cooperation.

We may text you a reminder of your appointment date and time, if you do not wish to receive these, please let us know in writing.

Please note we will be changing our clinical software on 31st May 2017.  This will inevitably result in some disruption of service but we will do our best to keep this minimal.  Please bear with us during this time and do not leave any repeat prescriptions to the last minute ! Thank you
